We believe in opportunities to promote healthier living and brighter tomorrows, which is why Quartz and Pivotal Health are partnering to encourage Quartz and Quartz Medicare Advantage (HMO) members to schedule a no-cost, virtual or in-home health and wellness appointment where a certified provider comes directly to you! 

Conveniently in the comfort of your own home, or as a scheduled video call, you’ll receive dedicated one-on-one time with an experienced and licensed provider to discuss important health topics, such as your medical history, health concerns, and health goals.

Benefits of this appointment include:

  • Extended visit time to discuss your personal health and wellness with a provider
  • A documented, post-assessment summary to be provided after the appointment
  • An extra appointment in addition to your usual annual primary care visit
  • Available flu shots at no cost

Following your health and wellness appointment, all information will be available to you and your primary care provider for follow-up discussions. We value your path to well-living and we’re confident this appointment will provide a convenient opportunity to help manage your health and wellness.

Is it safe to have the practitioner in my home?

Highly qualified Pivotal Health practitioners take all necessary steps to ensure your safety. Practitioners are background-checked, licensed, vaccinated, routinely tested for COVID-19, and use gloves and face masks. For optimum health safety, we recommend you wear a face mask, if possible. Pivotal Health also offers virtual visits.

What is the benefit of this visit?

The appointment allows for an unhurried visit, including discussing your prescriptions, medical history, and current health, with time to answer your questions. Quartz sends a follow-up letter to your doctor to support your care. This extended appointment lasts approximately 45 minutes to gather comprehensive information about your health and ensure you’re enrolled in all the available services that might benefit you. This preventive service is covered under your Quartz insurance plan. Quartz’s main goal is to provide the best care for you, including identifying your needs with a thorough annual assessment that goes to your physician.

I already have a doctor, so why should I do this visit?

The assessment helps to identify your current health status and needs. If you schedule a visit with your physician after the home assessment, your provider can spend more time discussing the management of your care rather than spending valuable time answering basic questions. This house call benefit does not replace your doctor’s care — it supplements it. During the house call, the provider will not change any medications or treatment plans set up by your doctor. However, a summary report from this visit will help your physician evaluate your current healthcare status and needs. Everything discussed will be shared with your doctor. This helps to create a personalized care plan just for you, as well as identify whether there are any programs or medical equipment that may be helpful to you. It also helps you to make the most of the benefits offered by your Quartz Health Insurance Plan.

Will I receive a copy of my medical record from the evaluation?

You will receive a follow-up after-visit summary by email or physical mail summarizing the results of the visit.

Who is Pivotal Health?

Quartz contracted providers at Pivotal Health to provide the Health and Wellness visits, so your house call visit will be with a licensed Pivotal Health practitioner. Pivotal Health is a healthcare provider based in Wisconsin specializing in in-home care. You’ll be seen by one of our Nurse Practitioners or Physician Assistants. Pivotal Health is highly rated for providing excellent, convenient care at home. They are certified medical professionals and will provide a complete summary of the visit to your provider and Quartz.

What happens during this visit?

This comprehensive visit includes screenings for vision, hearing, cognition, depression, injury risks, behavioral risks, and home safety. During your visit, please update your Pivotal Health provider about your current medications, allergies, and any specialists you see. The provider will check your vital signs and take notes on your medical history and relevant family history. You can also get your flu vaccine or other needed vaccines during the visit. Your Pivotal Health provider is prepared to discuss other areas to help you improve your quality of life, such as food planning, stress factors around your home, and a review of medications in your home. The Pivotal Health clinician can answer any questions you may have.

What do I need to do to prepare for the home visit?

Please have your current medications, pen and paper, and any questions available for the practitioner.

Do I need to fast?

No. You do not need to fast.

Is this a scam?

No. The best way to confirm that this is not a scam is to call the Quartz Customer Success team to ensure that Pivotal Health is authorized to perform this visit. You can find that number on the back of your member ID card.

Is there a co-payment or charge for this visit?

This service is covered as a preventive service under your Quartz Health Insurance Plan. That means there is no cost to you (no co-pays or deductibles) for the Health and Wellness assessment.

Am I required to participate?

We believe you would benefit from the visit to ensure the highest quality care for your individual health history. However, the visit is strictly voluntary.
