SMS Terms of Service

  1. In support of our services, Pivotal Health offers access to appointment reminders, post-appointment follow-up messages and other health programs via SMS text messages. By consenting to receive communications from Pivotal Health you are agreeing to receive these SMS messages from us regarding our healthcare-related services. Pivotal Health has multiple SMS programs:
    • Piv Hlth Reminders – Short Code Program
      • Pivotal Health uses this SMS program for:
        • Consent and Acknowledgment of Service
        • Appointment and service reminders
        • Post-Visit Surveys
      • Message Frequency: Pivotal Health will send no more than 2 reminder messages per month excluding scheduled visit reminders.
    • Piv Hlth Logistics – Long Code Program
        • Pivotal Health uses this SMS program for:
          • Provider logistics
          • Links to required forms
          • Billing questions and follow-up
        • Message Frequency: Pivotal Health will send messages according to the number of visits you have scheduled.
  2. Opt-Out Information: You can cancel the SMS services at any time. Just text “STOP” to our texting number. After you send the ” STOP ” message to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us.
  3. Get Help or Support: If you are experiencing issues with the messaging program you can reply with the keyword HELP for more assistance, or you can get help directly at 888-688-4746 or by emailing info@pivotalhealth.care.
  4. Carriers are not liable for delayed or undelivered messages.
  5. As always, message and data rates may apply for any messages sent to you from us and to us from you. You will receive messages via this SMS short code according to the communications permissions you have provided to us; message frequency will vary with the number of appointments you schedule. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.

This SMS Privacy Policy is intended to inform you of Pivotal Health’s policies and practices regarding the collection, use and disclosure of any information you submit to us in connection with receiving text messages. By scheduling an appointment and/or providing consent to be contacted by us, you agree to the terms and conditions of this SMS Privacy Policy and the associated SMS Terms of Service. See above. As a healthcare provider, we are committed to protecting your health information and are bound by federal and state law to do so. Any information we collect from you using text messages will be used solely for healthcare related services, contact you have otherwise agreed to, and other lawful purposes (including your treatment, payment, and our healthcare operations). We will never share or disclose any of your data with third parties except in accordance with state and federal laws including but not limited to the federal Health Insurance Portability and Accountability Act (“HIPAA”).
