Conveniently in the comfort of your own home, or as a scheduled video call, you’ll receive dedicated one-on-one time with an experienced and licensed provider to discuss important health topics, such as your medical history, health concerns, and health goals.

We can evaluate and treat pain, congestion and fevers. This includes rapid strep, flu, and rapid COVID testing as needed, with results at home within minutes! Your cost is $169.
We can evaluate and treat cough illnesses. This includes RSV, flu and rapid COVID testing as needed, with results at home within minutes! If it's appropriate, we'll also arrange for x-rays to be done right in your home. If you know you'll want chest x-rays for sure, please let us know so we can arrange for our radiology assistant to meet us at your home! All x-rays are interpreted by our provider on-site and a radiologist remotely. Your cost is $169 with an additional $169 if x-rays are needed.
We can evaluate and treat simple infections, remove foreign objects like cotton swab tips and flush excess wax from the ears. Your cost is $169.
We can evaluate and treat dental pain with topical treatments, temporary fillings, temporary injection nerve blocks (if desired!) drainage of abscesses and antibiotics to get you through to a dental appointment. If you're looking to get a tooth pulled, a root canal or a more advanced procedure, you'll need to contact a dentist. Your cost is $169.
We can help by evaluating the eye, treating infection and pain, removing a foreign body if needed and recommending further eye care if needed. If you have sudden loss of vision for any reason, we'd recommend calling 911 or having someone take you directly to the nearest ER. Your cost is $169.
We can evaluate and treat new-onset nausea, vomiting and/or diarrhea with medications. If needed or desired, we can start an IV and give fluids/medications that way. If it's appropriate, we can arrange for basic x-rays to be performed right in your home that are interpreted by our provider on-site and by a radiologist remotely! If you think you might need/want an ultrasound or a CT scan, you should go directly to the nearest ER. Otherwise, we can see you and then recommend further treatment in the ER or with a specialist as needed.. Your cost is $169 and an extra $69 for IV or lab testing and an additional $169 if x-rays are needed.
We can evaluate and treat bladder infections and common sexually transmitted infections like gonorrhea and chalmydia by doing a quick urine test and getting you antibiotics right away if needed. For an additional fee, we can draw blood as needed or collect samples for send-out tests such as HIV, hepatitis, herpes and others. Your cost is $169 and an extra $69 if an IV or lab tests are required.
We can evaluate and treat most new rashes (less than two weeks), bug/tick bites, and simple dog/cat bites. If you are feeling any severe allergic reaction like tongue swelling or difficulty swallowing/breathing or any signs of significant illness like fevers or large areas of redness on the skin, you should call 911 or go directly to the nearest Emergency Room. Your cost is $169.
We can evaluate and repair simple cuts with sutures or skin glue if the injury has occurred in the last 8 hours. If your cut is close to the eye or if there is concern about damage to bones, tendons or other deep structures, we'd recommend going to the nearest Emergency Room. Your cost is $169.
We can evaluate and treat most nail problems, like painful ingrown nails (hangnails), infections around the nail or nail injuries. Your cost is $169.
We can evaluate and treat most boils (abscesses) with a minor procedure (incision and drainage) with or without antibiotics. If the infected area is larger than the palm of your hand or if you have fevers, we'd recommend going to the nearest emergency room. Your cost is $169.
We can evaluate and treat most puncture injuries where something like a sliver or fish hook is still in or under the skin. Your cost is $169.
We can evaluate and treat simple tension and migraines headaches with by-mouth medications and/or IV medications and fluids as needed. If this is the worst headache of your life, you have fever or have had a recent head injury, we'd recommend going to the urgent care or emergency room. Your cost is $169.
If you DON'T have COVID symptoms but are concerned about exposure or need clearance for travel clearance or return to work/school, we can do a rapid antigen test with results within 15 minutes, PCR tests with results in within 72 hours. If you do have symptoms and would like an evaluation as well, we can perform that in the same visit.

We offer hassle-free school/sports physical exams along with pre-employment physical exams with completion of all necessary forms in the convenience of your home. We can also draw . Your cost is $169.
We can evaluate and treat most common injuries and joint pain. We'll do a focused exam and, if appropraite, arrange for x-rays to be done right in your home. If you know you'll want xrays for sure (concerned about rib fractures, broken wrist or ankle), please let us know so we can arrange for our Xray team to meet us at your home! All x-rays are interpreted by our provider on-site and a radiologist remotely. Then, we will provide pain control, wound care, injury splinting, plan for specialist follow-up and whatever else is needed for a safe and speedy recovery. Your cost is $169 with an additional $169 if an x-ray is required.
We can come to your home and draw blood for most screening lab tests (complete blood count, metabolic panels, thyroid function, hemoglobin A1c for diabetes, cholesterol panel, you name it!). Most results are available within 24 hours!. Your cost is $99.
We can come to your house, place a PPD test in the skin and then follow up with you by brief video call in 2-3 days to read the test and complete your forms! Your cost is $169.
We can arrange a video/phone visit with a Pivotal provider for professional advice about a medical problem. We'll do our best to help resolve the issue virtually. If the problem requires an in-person visit, we can schedule a same-day house call with a Pivotal provider or help get you to the nearest emergency room if it's a true emergency. Your cost is $69.

Is it safe to have the practitioner in my home?

Highly qualified Pivotal Health practitioners take all necessary steps to ensure your safety. Practitioners are background-checked, licensed, vaccinated, routinely tested for COVID-19, and use gloves and face masks. For optimum health safety, we recommend you wear a face mask, if possible. Pivotal Health also offers virtual visits.

What is the benefit of this visit?

The appointment allows for an unhurried visit, including discussing your prescriptions, medical history, and current health, with time to answer your questions. Quartz sends a follow-up letter to your doctor to support your care. This extended appointment lasts approximately 45 minutes to gather comprehensive information about your health and ensure you’re enrolled in all the available services that might benefit you. This preventive service is covered under your Quartz insurance plan. Quartz’s main goal is to provide the best care for you, including identifying your needs with a thorough annual assessment that goes to your physician.

I already have a doctor, so why should I do this visit?

The assessment helps to identify your current health status and needs. If you schedule a visit with your physician after the home assessment, your provider can spend more time discussing the management of your care rather than spending valuable time answering basic questions. This house call benefit does not replace your doctor’s care — it supplements it. During the house call, the provider will not change any medications or treatment plans set up by your doctor. However, a summary report from this visit will help your physician evaluate your current healthcare status and needs. Everything discussed will be shared with your doctor. This helps to create a personalized care plan just for you, as well as identify whether there are any programs or medical equipment that may be helpful to you. It also helps you to make the most of the benefits offered by your Quartz Health Insurance Plan.

Will I receive a copy of my medical record from the evaluation?

You will receive a follow-up after-visit summary by email or physical mail summarizing the results of the visit.

Who is Pivotal Health?

Quartz contracted providers at Pivotal Health to provide the Health and Wellness visits, so your house call visit will be with a licensed Pivotal Health practitioner. Pivotal Health is a healthcare provider based in Wisconsin specializing in in-home care. You’ll be seen by one of our Nurse Practitioners or Physician Assistants. Pivotal Health is highly rated for providing excellent, convenient care at home. They are certified medical professionals and will provide a complete summary of the visit to your provider and Quartz.

What happens during this visit?

This comprehensive visit includes screenings for vision, hearing, cognition, depression, injury risks, behavioral risks, and home safety. During your visit, please update your Pivotal Health provider about your current medications, allergies, and any specialists you see. The provider will check your vital signs and take notes on your medical history and relevant family history. You can also get your flu vaccine or other needed vaccines during the visit. Your Pivotal Health provider is prepared to discuss other areas to help you improve your quality of life, such as food planning, stress factors around your home, and a review of medications in your home. The Pivotal Health clinician can answer any questions you may have.

What do I need to do to prepare for the home visit?

Please have your current medications, pen and paper, and any questions available for the practitioner.

Do I need to fast?

No. You do not need to fast.

Is this a scam?

No. The best way to confirm that this is not a scam is to call the Quartz Customer Success team to ensure that Pivotal Health is authorized to perform this visit. You can find that number on the back of your member ID card.

Is there a co-payment or charge for this visit?

This service is covered as a preventive service under your Quartz Health Insurance Plan. That means there is no cost to you (no co-pays or deductibles) for the Health and Wellness assessment.

Am I required to participate?

We believe you would benefit from the visit to ensure the highest quality care for your individual health history. However, the visit is strictly voluntary.
